If you have been putting off care because you heard root canals are painful, you are not alone. That fear is common and mostly outdated. With modern local anesthesia, root canal treatment is designed to relieve tooth pain, not cause more of it.
This guide explains why a tooth may need a root canal, the signs to watch for, what treatment generally feels like, and why getting a painful tooth checked early matters.
Inside every tooth is soft tissue called the pulp, made of nerves and blood vessels. When decay, a crack, or trauma reaches the pulp, bacteria can cause inflammation or infection. Left untreated, infection can spread to the bone around the tooth and lead to an abscess.
A root canal removes the damaged pulp, then cleans and seals the inside of the tooth. Saving a natural tooth is often the better choice over extraction when the tooth can still be restored.
Not every infected tooth hurts badly at first. If you notice any of these signs, schedule an exam rather than waiting. For urgent problems, see our emergency dental care page.
The tooth and surrounding gum are numbed thoroughly before treatment begins. Most patients feel pressure or vibration rather than sharp pain, and more anesthetic can be given if anything feels uncomfortable.
Mild soreness for a few days afterward is common as the tissues settle. Follow the aftercare instructions you are given, and seek care if pain increases, swelling grows, or you develop a fever.
Teeth that have had a root canal, especially back teeth, often need a dental crown to protect them from cracking and restore normal chewing.
The toothache that brings people in (throbbing pain, sensitivity to hot drinks, pain when biting) is usually worse than root canal treatment itself. Waiting can let infection spread, cause swelling or an abscess, and leave fewer options for saving the tooth.
At MK Dental Studio in Fulshear, TX, Dr. Madiha Khanani, DDS, FAGD, can examine the tooth, take X-rays, find the cause of your pain, and explain your options clearly. Not every toothache means a root canal; sometimes a filling or other treatment is enough.
Dental anxiety is real, and it is welcome to talk about it. MK Dental Studio offers nitrous oxide (laughing gas) to help anxious patients feel calm.

This article is educational and is not a diagnosis. Only an in-person exam and X-rays can show whether a root canal is right for your tooth.
